Appointment Booking Policy — Life Care Hospital & Maternity Home

Welcome to Life Care Hospital & Maternity Home. By booking an appointment through our online system, you acknowledge that you have read, understood, and agree to be bound by all of the following terms and conditions. Please read them carefully before proceeding with your booking.

1 General Terms

These terms govern the online appointment booking service offered by Life Care Hospital & Maternity Home (hereinafter referred to as "the Hospital", "we", "us", or "our"), located at Sector A GT Road, opposite Utman Marriage Hall, Naseem Town Darvesh, Haripur, KPK, Pakistan.

  1. Online appointments are available for OPD (Outpatient Department) consultations and diagnostic services only.
  2. Emergency services are available 24/7 and do not require pre-booking. Call 0995-321234 immediately in case of emergency.
  3. The Hospital reserves the right to modify, suspend, or terminate the online booking service at any time without prior notice.
  4. All services are subject to availability and the operating hours of the respective doctor or department.
  5. These terms are governed by the laws of the Islamic Republic of Pakistan.

2 Appointment Booking Process

To successfully book an appointment, you must:

  • Select a doctor and service type from the available options.
  • Choose an appointment date and time within the doctor's working hours.
  • Provide accurate patient details: full name, phone number, and address.
  • Complete the Easypaisa payment and provide your transaction ID.
  • Upload a screenshot or photograph of your payment confirmation.
  • Agree to these Terms & Conditions before submitting your booking.

Important: All fields in the booking form are mandatory. Incomplete or inaccurate submissions may result in cancellation without refund.

Upon successful submission, you will receive a Booking Receipt (PDF) with a unique booking reference number and status marked as "Pending". This receipt confirms your submission but does NOT guarantee a confirmed appointment until payment is verified by our team.

3 Doctor OPD Hours & Availability

Appointments can only be booked during the respective doctor's working hours. The current OPD schedule is as follows:

⚠ Friday & Sunday: OPD OFF

Dr. Bushra Sadia

Days: Monday to Saturday

Hours: 3:00 PM – 5:00 PM

⚠ Sunday: OPD OFF

✓ Emergency: 24/7 Available

  1. OPD hours are subject to change without prior notice due to medical emergencies, public holidays, or other circumstances.
  2. The Hospital will make reasonable efforts to notify patients of schedule changes via phone.
  3. Booking outside the designated OPD hours is not permitted through the online system.

4 Services & Pricing

The following services and fees apply at the time of booking:

  1. Prices are inclusive of all applicable taxes and charges at the time of booking.
  2. The Hospital reserves the right to revise pricing at any time. The price applicable is the one displayed at the time of booking.
  3. Additional charges may apply for services rendered beyond the booked scope during your visit.

5 Payment Terms

All appointment fees must be paid in full at the time of booking via Easypaisa:

  1. Payment must be completed in full before submitting the booking form.
  2. You must enter the correct Easypaisa transaction ID in the form.
  3. You must upload a clear, legible screenshot of your payment confirmation.
  4. Appointments will not be confirmed until payment is verified by our administrative team.
  5. Any false, fraudulent, or incorrect transaction information will result in immediate cancellation without refund.
  6. The Hospital is not responsible for payment delays, failures, or errors caused by Easypaisa or your mobile banking service.

Cancellation & Refund Policy

⚠ Please Read Carefully

Our cancellation and refund policy is strictly enforced. By booking an appointment, you agree to these terms unconditionally.

6.1 — Cancellation Window

✓ Eligible for Refund

Cancellation requested more than 12 hours before your scheduled appointment time.

✗ No Refund

Cancellation requested 12 hours or less before your scheduled appointment time.

6.2 — Refund Amount

When eligible, refunds are processed at 50% of the total amount paid. The remaining 50% is retained by the Hospital as an administrative and processing fee. No exceptions will be made to this policy.

6.3 — Cancellation Rules

  • No refund is provided for no-shows (failure to attend without cancellation).
  • No refund if appointment is cancelled within 12 hours of the scheduled time.
  • No refund for appointments cancelled after the scheduled time has passed.
  • No refund for bookings made with incorrect or fraudulent information.
  • To cancel, contact us by phone on 0334-3817670 or 0995-321234 and provide your booking reference number.
  • Refunds (where eligible) will be processed to the original Easypaisa account within 3–5 business days.
  • The Hospital may cancel an appointment at any time due to unforeseen medical circumstances or doctor unavailability, in which case a full refund will be issued.

6.4 — Rescheduling

Appointments may be rescheduled (subject to availability) by contacting the Hospital at least 12 hours in advance. Rescheduling is permitted once per booking at no additional charge. Subsequent rescheduling requests may incur an additional fee.

7 Patient Responsibilities

  1. Patients must arrive at least 10–15 minutes before the scheduled appointment time.
  2. Carry a printed or digital copy of your booking receipt and a valid CNIC or photo ID.
  3. Inform the Hospital immediately (at least 12 hours in advance) if you are unable to attend.
  4. Provide accurate, complete, and truthful information in the booking form. Any false information may result in appointment cancellation without refund.
  5. The patient (or their guardian) is responsible for disclosing relevant medical history and current medications to the doctor.
  6. Patients must behave respectfully toward hospital staff and other patients. The Hospital reserves the right to refuse service to any individual displaying disruptive or abusive behaviour.
  7. Minors must be accompanied by a parent or legal guardian at all times during the appointment.

8 Privacy & Data Protection

We are committed to protecting your personal information. The data collected during the booking process (name, phone number, address, payment details) is used solely for appointment management purposes.

  1. Your personal data will not be sold, rented, or shared with third parties for commercial purposes.
  2. Payment screenshots and transaction IDs are stored securely and used only for payment verification.
  3. Patient medical information is confidential and handled in accordance with professional medical ethics.
  4. By submitting the booking form, you consent to the Hospital contacting you via phone for appointment-related communications.

9 Limitation of Liability

  1. Life Care Hospital & Maternity Home provides this online booking service "as is" and makes no warranties regarding uninterrupted or error-free operation.
  2. The Hospital shall not be liable for any indirect, incidental, or consequential damages arising from your use of the online booking system.
  3. In the event of technical failure during booking, contact the Hospital directly by phone to complete your appointment.
  4. The Hospital's liability in any case shall not exceed the amount paid for the appointment being disputed.

We aim to resolve all complaints within 48–72 hours of receipt. If a satisfactory resolution cannot be reached, disputes shall be settled through mediation under Pakistani law in the jurisdiction of Haripur, KPK.

By checking the "I agree to the Terms & Conditions" box in the appointment booking form and submitting your booking, you confirm that you have read, understood, and agree to all terms outlined in this document.
